When should PRP be considered a form of treatment for arthritis of the knee?

At present the evidence is as follows:

  1. Patients with established OA are pain free on average for 9 months after 3 injections at 4 week intervals. More the deformity less the benefit: hence I put on them a very strict aftercare by good physios to strengthen the soft tissue supporting structures eg: quads etc.
  2. Young patients with no OA but chondral lesions needing arthroscopy: Do better with PRP than the older patient with OA!
  3. I inform patients of omega 3 fatty acids, blueberries, vit D and green tea for 4 weeks prior to PRP to replenish autologous stem cells.

When and how would I offer joint PRP? when patients have pain, disability, do a pre treatment oxford score, get them on oral supplements,for 4 weeks and away you go!
